Why Healthcare Costs Vary So Much by State
Healthcare costs in the United States are deeply local. The same medical procedure can cost 2-5 times more in one state compared to another, and health insurance premiums vary by hundreds of dollars per month depending on where you live. Several factors drive these differences:
State Marketplace Structure
Some states operate their own ACA marketplaces (like Covered California or NY State of Health), while others use the federal HealthCare.gov platform. State-run marketplaces often have more insurer competition and lower average premiums due to active management of plan offerings.
Medicaid Expansion Status
As of 2025, 40 states have expanded Medicaid under the ACA, covering adults up to 138% of the Federal Poverty Level. In the 10 states that haven't expanded, many low-income adults fall into a "coverage gap" where they earn too much for Medicaid but too little for marketplace subsidies.
Provider Market Concentration
States where a few large hospital systems dominate the market tend to have higher prices because hospitals have more negotiating power with insurers. States with more competition among providers typically see lower procedure costs and more affordable insurance premiums.
Cost of Living & Labor Costs
Healthcare worker salaries, facility costs, and overhead vary dramatically by state. A nurse in San Francisco earns nearly double what a nurse in Mississippi earns, and those costs are reflected in healthcare prices charged to patients and insurers.
